Organ failure scale adapted for severe Cuban pediatric patients with acute lymphoid leukemia

Introduction: The pediatric organ failure scale is a tool used in pediatric intensive care units. However, since its initial validation, its parameters have not been modified, when there are different groups of diseases and patients.
Objectives: Validate the pediatric organ failure evaluation scale adapted for severely ill Cuban patients with a diagnosis of acute lymphoid leukemia.
Methods: An observational, prospective, multicenter study was carried out in pediatric intensive care units of Cuban hospitals with 184 patients. The following variables were analyzed: demographics, presence of organ failure, platelet count, clotting times, and status at discharge.
Results: The pSOFA score with adaptation in the hematological component was higher in the deceased (p < 0.001) and mortality increased progressively in patients with platelet count less than 20 x 106 and increased clotting times. Receiver operating characteristic curve analysis showed that the area under the curve for predicting mortality with the pSOFA score with adaptation in the hematological component was 0.92.
Conclusions: The use of the pSOFA scale with modification of the hematological component obtained greater specificity to predict mortality compared to the original scale.
