Resumen

El manejo en la etapa aguda de la enfermedad cerebrovascular amerita que los médicos que brindan soporte a estos pacientes tengan conocimiento de mecanismos fisiológicos básicos. La autorregulación cerebral y la perfusión pueden estar alterados por la evolución en las primeras etapas de la enfermedad.  Episodios hipotensivos pueden ser deletéreos para nuestros pacientes. Si bien la literatura reciente no soporta beneficios en relación con la disminución de la presión arterial en eventos isquémicos y hemorrágicos, el manejo aún debe continuar adherido a las guías de manejo disponibles. Se presentan algunos aspectos relevantes de este tópico motivo de discusión en neurointensivismo.
